We work with a variety of insurance plans.

Please contact the office if you have an insurance not listed on our website. We can verify if we are contracted with your particular insurance company.

Certain insurances, such as HMOs and AHCCCS, require that you designate us as the primary care physician. Please check with your insurance company if this is a requirement.

Your insurance will be verified at every office visit. Please notify the front desk if there have been any changes to your insurance policy.

Copayment (“copay”) is collected at the time of service, based on your insurance rules. For your convenience, we accept cash, credit cards (Master/Visa) or personal checks. If you do not have insurance, payment in full is required at the time of service. Please see our billing policy for more details.

If you have a newborn, you are required to add the baby to your insurance policy, usually within the first month. This is your responsibility.

Our office billing is handled by eBill Medical Solutions. You will be directed to contact 480 792 1080 for all billing enquiries.
